Georgia Department of Behavioral Health and Developmental Disabilities

HOW TO APPLY FOR SERVICES

Adults aged 18 and older with developmental disabilities are eligible to apply for Medicaid Waiver services through the Department of Behavioral Health and Developmental Disabilities (DBHDD). Eligibility is initially handled through DBHDD and all individuals interested in services will need to be placed on the state planning list which is administered through DBHDD. This web-based portal through the Georgia Collaborative allows individuals and families to submit applications for IDD services and track their application through the process in real time. Individuals and their families can view their application determination online and submit secure messages regarding their application.

Items that will be needed to submit your online application include:

  • Official Birth Certificate

  • Medicaid/Medicare Card

  • Psychological Evaluation

  • Social Security Card

  • Developmental Evaluation

  • School IEP Report

  • School Transcript

  • Vocational/Support Employment Records

  • Permanent Residence Card

  • Social Security Benefit Letter

  • Other Medical or Diagnostic Reports

DCRA does not participate in the Waiver application and approval process.  Once an individual is determined to be eligible and most in need by the state office of DBHDD, the individual may choose to participate in Waiver Services via the DCRA.
